About Diabetes Management and Education Programs and Support Services:
For people living with type 2 diabetes, there are a set of services and support programs to help them manage their disease. These includes ADCES- accredited and ADA-recognized Diabetes Self-Management Education and Support services (DSMES), the Diabetes Self-Management Program (DSMP), and DEEP™. The programs help participants control their diabetes and prevent complications.
The Initiative:
The Cook County Collaboration to Advance Reach, Equity and Systems (CARES: Cook County) program will help expand access to diabetes prevention, diabetes management and family-focused healthy lifestyle programs in Cook County, IL priority communities. Program partners will help implement evidence-based programs tailored to meet the needs of their local communities.
CARES Cook County is led by the Illinois Public Health Institute with support from the Centers for Disease Control and Prevention.

Take Charge of Your Diabetes, a DSMP program of the Illinois Pathways to Health by AgeOptions.
This program helps people with type 2 diabetes take better control of their diabetes through weekly workshops that address nutrition and healthier eating, physical activity, medication management, and more.

DEEP™ is a diabetes self-management education (DSME) program that has been shown to be successful in helping participants take control of their diabetes and reduce the risk of complications. DEEP™ was developed for use in low-income, racial and ethnic minority populations.
